A tourism board representative admitted lapses in internal communication, telling reporters, “We are examining every protocol to ensure this never recurs.”

Warner, beloved for his Emmy-nominated role on “The Cosby Show,” was on vacation celebrating a recent theater debut. Witnesses say he waved off warnings, confident he could handle the surf. When he failed to return, bystanders flagged down a passing boat, but emergency services arrived too late to reverse the effects of the powerful currents.

Investigators are now sifting through public-record duty rosters and time sheets to pinpoint who authorized leaving the station vacant. The Costa Rican Tourism Board has promised an independent audit and plans to double lifeguard coverage at all major beaches by next high season.
